Chapter 1: Understanding HIV Tests
1.1 What You Are Actually Measuring
Every HIV test, regardless of its shape, brand, or price, is a detector. It is designed to detect one of two things: a piece of the virus itself, or a molecule that your body's immune system has created in response to that piece of the virus. This distinction is the single most fundamental concept in HIV testing, and it drives every operational decision you will make, from choosing a kit to interpreting its result.
When a person is infected with HIV, the virus begins replicating. The body's immune system recognizes the viral proteins, or antigens, as foreign. In response, it begins producing antibodies. These two events do not happen simultaneously. The virus itself is present from the moment of infection, and its RNA can be detected within days. However, it can take several weeks for the immune system to produce enough antibodies to be detected by a test. This period, between infection and the point at which a test can reliably detect it, is called the window period. The length of this window is the most important specification you will consider when selecting a test.
To understand this practically, consider a typical scenario. A person comes to a walk-in clinic, reporting a potential exposure three weeks ago, worried about their status.
