Chapter 1: Understanding Ice Packs

An ice pack is, at its most basic, a container for a cold substance that you apply to a surface to remove heat from it. In the context of this handbook, that surface is usually human tissue, but the same principles apply whether you are protecting a shipment of vaccines, packing a cooler for a fishing trip, or keeping a horse's leg cool after a training session. The purpose is always the same: to transfer thermal energy from the warm object to the cold object. Understanding that the function is thermal transfer, not just "putting ice on it," is the first step toward using ice packs correctly.

When you place an ice pack on a sprained ankle, you are not actually "freezing" the ankle. You are drawing heat out of it. Heat always flows from warmer materials to colder materials. The skin, at roughly 33°C, is warmer than the ice pack, which is at 0°C for an ice-water mixture. The heat from the skin and the underlying tissue flows into the pack, which is why the pack gradually warms up. The goal of cryotherapy, or cold therapy, is to lower the temperature of the injured tissue enough to reduce metabolism, slow down the inflammatory response, and numb the nerve endings to relieve pain. This is a controlled physiological intervention, not merely a matter of comfort.
