What a Stump Shrinker Is and Why It Is Not a Sock
In the occupational vocabulary of prosthetics, the word "sock" is used loosely, often to describe any textile layer placed between a residual limb and its socket. A stump shrinker is not a sock. While a prosthetic sock primarily acts as a volume filler to take up slack in a properly fitting socket, a stump shrinker is an active compression device used during the period before a socket is fabricated, and sometimes in a mature limb to manage volume fluctuations. It applies a deliberate, distributed, and medically significant pressure gradient from the distal end of the limb, nearest the surgical site, to the proximal region, closest to the body's trunk. This pressure gradient is the core of its function: it controls edema, or swelling, and shapes the soft tissue into a volumetric cone that is suitable for bearing weight and distributing pressure inside a rigid socket.
Consider the clinical reality of a transtibial (below-knee) amputation. A surgeon may perform the myodesis, cutting and stitching muscles to the bone, to create a stable lever arm for weight-bearing. Immediately after closure, the residual limb is swollen with extracellular fluid, it is warmer than the rest of the body, and its volume is at its largest and most unpredictable. Suture lines are healing, and the bone that has been severed is still sharp and acutely sensitive. This is the hostile environment where the stump shrinker does its
